To enable learners to demonstrate knowledge, skills and attitudes consistent with professional standards in the provision of person-centred care to people with acute and chronic health Conditions.

Course learning outcomes

  • In collaboration with relevant inter-disciplinary team members, conduct a comprehensive assessment and document a plan of care for a person
  • Implement safe and effective person-centred nursing care to achieve the identified health outcomes.
  • Use problem solving skills to evaluate the effectiveness of the care plan and revise where necessary to meet the continuing needs of the person/group
  • Demonstrate appropriate communication strategies to promote an optimal physical, psychosocial, cultural and spiritual environment for people/groups.
  • Apply analytical skills in accessing health information and research evidence in the provision of person-centred nursing care. 
  • Evaluate own nursing practice in accordance with professional standards, legal and ethical policies and codes of practice and revise where appropriate.
