- Conduct a health assessment and document a person-centred plan of care for an older person
- Implement safe and effective person-centred nursing care to achieve identified health outcomes consistent with relevant legislation and professional standards
- Develop, evaluate and revise a plan of care to meet continuing needs of the older person
