Use this form to request changes to your personal details. Note that changes to your DOB must be made in-person at Student Assist with appropriate documentation.

This form supports a student's application to change their study arrangements at Adelaide University due to health-related circumstances, and must be completed by a registered health care professional within their scope of practice.

This form is to be used by students with a diagnosed disability, ongoing medical or health condition or who are the carer for a person with a documented disability or health condition, who seek reasonable adjustments to their studies.

This form is to be used by student to book appointments with specialised student support services, including Counselling, Access & Inclusion, Student Academic Learning Support, International Student Support and First Nations Student Support.

This form is used by students to request a letter from Counselling to support an assessment extension or replacement exam.

Use this form to apply for credit into your Adelaide University program for your previous studies (whether at Adelaide University or at a different institution) or for informal credit.

Request a leave of absence from your Adelaide University program.

Request a Leave of Absence (LOA) from your Adelaide University program. This form is to be completed by international students on a Student Visa (subclass 500).

Request a program withdrawal from your Adelaide University program.

Request a Withdrawal from Program from your Adelaide University program. This form is to be completed by international students on a Student Visa (subclass 500). 

This form is to be completed by international students on a Student Visa (subclass 500) seeking to transfer to another Australian registered education provider prior to completing six (6) calendar months of study in their principal program at Adelaide University.

This form is to be completed by international students on a Student Visa (subclass 500) seeking a Confirmation of Enrolment (CoE) extension.

This form is to be completed when an international student is unable to study a full time load due to exceptional circumstances.

This form is to be completed by international students on a Student Visa (subclass 500) seeking to apply for a 1545 Visa Application Fee Waiver.

This form is used by international students to request a letter of support from the International Student Support team. This form is not to be used by students to request a letter confirming the University's official teaching breaks.
